Cold hands, thinning hair, flat energy, a body that gains easily and loses reluctantly, and a single TSH result sitting inside the range. A normal TSH doesn’t rule out a struggling thyroid, and it certainly doesn’t explain away how you feel.

Beyond a lone TSH

A TSH alone is a smoke alarm, not a full picture. I test beyond it: free T4 and free T3 to see actual hormone availability, thyroid antibodies to catch Hashimoto’s early, and the nutrient cofactors the thyroid depends on, iron, iodine status, selenium and zinc among them. The pattern across those markers tells a far more honest story than one number inside a wide range.

Treatment depends on what turns up. That may mean nutrient repletion in therapeutic forms, herbal and lifestyle support for the stress physiology that drags thyroid conversion down, and immune-focused care where antibodies are in the picture, all alongside your GP, including if you’re on or considering thyroid medication.

This is one of the most common presentations in my clinic. A full panel including free T3, free T4 and antibodies, read alongside your symptoms and nutrient status, often reveals a picture a lone TSH misses. Where everything truly is optimal, I keep investigating rather than sending you away with nothing.

Do you treat Hashimoto’s?
